At The Ultimate Care, our caregivers are carefully selected, not only for their skills but for their compassion and empathy. Each caregiver is trained to provide the highest level of personalized care, ensuring that clients receive attention that uplifts their spirit and enhances their daily life.

Tier 1: Basic Care (Quick Services)

Hours: 1-6 hours per day


Services Include:

  • Activities of Daily Living

    • Bathing

    • Dressing

    • Toileting

    • Grooming

  • ​Companion Services

  • Social Engagement

  • Focus Client Collaboration Meetings (FCCM) - 2 times per year

  • Quarterly Supervisory Visits

Tier 2: Enhanced Care (Core Services)

Hours: 7-11 hours per day


Services Include:

  • All Tier 1 Services*

  • Medication Reminders

  • Errand Services

  • Basic Health Monitoring (vital signs checks)

  • Basic Medical Tasks (blood sugar checks)

  • Telehealth Services (quarterly check-in)

  • Nutrition and Meal Preparation

  • Light Housekeeping and Laundry

  • Focus Client Collaboration Meetings (FCCM) - 3 times per year

  • Quarterly Supervisory Visits

Tier 3: Premium Care (Advanced Services)

Hours: 12-19 hours per day


Services Include:

  • All Tier 1 and Tier 2 Services*

  • Telehealth Services (bi-monthly check-in)

  • Specialized Services (palliative, hospice, pediatric)

  • Comprehensive Care Management

  • Priority Scheduling and Emergency Response

  • Focus Client Collaboration Meetings (FCCM) - 4 times per year

  • 60-Day Supervisory Visits

  • Transportation Services to Medical Appointments and Social Events

Tier 4: Concierge Care (Specialized Services)

Hours: 20 hours or more per day


Services Include:

  • All Tier 1, Tier 2, and Tier 3 Services*

  • Personalized Care Coordination

  • Dedicated Care Team

  • Enhanced Communication and Support

  • Focus Client Collaboration Meetings (FCCM) - 6 times per year

  • Monthly Supervisory Visits

  • Telehealth Services (monthly check-in)

  • In-Home Doctor Visits

  • In-Home Lab Draws

  • Medication Management

  • Medical Social Services
