Position Summary:
As a Registered Nurse (RN) or Licensed Practical Nurse (LPN), you will provide supplemental and contract services to our clients in a compassionate, professional, ethical, and knowledgeable manner. You will work under the direct supervision of the Director of Nursing at the assigned home and understand the limitations and scope of care that can be provided based on your specific education level. You will collaborate closely with the Director of Nursing to deliver the nursing process effectively.
Reporting Requirement:
To Clinical Nurse Director. The RN / LPN will work under the supervision of a Clinical Nurse Director and will report any changes in the patient's condition.
As a Registered Nurse (RN) or Licensed Practical Nurse (LPN), you will be responsible for:

Assessing, observing, and speaking to patients.

Recording details and symptoms of patient medical history and current health.
-
Preparing patients for exams and treatment.
-
Administering medications and treatments and monitoring patients for side effects and reactions.

Creating, implementing, and evaluating patient Plan of Cares with the medical team.
-
Performing wound care, such as cleaning and bandaging.
-
Assisting in medical procedures as needed.
-
Operating and monitoring medical equipment.
-
Drawing blood, urine samples, and other body fluids for lab work.
-
Educating patients and family members on treatment and Plan of Cares, and answering their questions.

Supervising licensed practical nurses, nursing assistants, and nursing students.

Expected to perform all nursing duties in accordance with federal, state, VA, and local laws, while adhering to all facility policies and procedures.

Provide routine nursing care for assigned patients, using age-specific and universal precautions at all times, and practice patient teaching and training for patients and families according to the Plan of Care.

Follow hospital policy for disposing of biohazard materials and properly label and handle all materials.

Administer prescribed medications to assigned patients orally, topically, subcutaneously, intradermally, and intramuscularly under the direct supervision of the Director of Nursing, being knowledgeable of all medications, reactions, and dosage requirements.

Communicate with all staff, patients, and families using excellent written and verbal skills and consistently seek educational tools and in-services to broaden your knowledge of various age groups, diseases, and best practices.

Handle and be knowledgeable about all medical equipment used in patient care and perform various sensory requirements such as vision, speech, smell, touch, manual dexterity, fine motor skills, and hearing in order to accurately assess and treat the patient.

Comply with all facility policies and procedures, upholding the high expectations of Ultimate Care, LLC, and be accountable and responsible for each specific patient assigned while under direct supervision.

Be punctual and dependable for assigned and confirmed shifts.
Requirements:

One year or more of full-time clinical practice in each specialty area (Preferred).

To be considered for this position, you must have graduated from an accredited College of Practical Nursing and hold a current nursing license in the state(s) in which you wish to practice.

Current BLS/CPR training with card, any other specialty certifications required for working in specific specialty areas (PALS, ACLS, NRP, etc).

PPD / TB test.

Ability to read, write, and speak English proficientlyStrong communication and interpersonal skillsPhysical ability to lift and move patients, as needed. Able to lift up to 50 poundsValid driver's license and reliable transportation (preferred).
Working Conditions:
This is a full-time or part-time or prn position, with hours varying depending on the needs of the patient.
The RN / LPN may work evenings, weekends, and holidays as needed.
The RN / LPN will work in the patient's home, and may be required to travel to different locations within the service area.
Compensation:
The RN / LPN will be paid a competitive hourly wage, with the rate depending on experience and location.

$35.00 - $55.00 per hour
